Plan mode has become unreliable

### Describe the bug

The last couple of weeks planmode starts executing the plan randomly. Sometimes it waits for acceptance, as it is supposed to, but other times it asks me but continues straight away without waiting for answer. I don't see any pattern in it.

I'd expect Plan mode to always **_ONLY_** create a plan

### Steps to reproduce the behavior

Plan, execute, plan etc. The it will happen at somepoint

### Expected behavior

Plan mode always waits for accept before starting autoimplement

The issue names no files or tests. Reproduce the Plan/execute/plan sequence on version 1.0.57 and observe whether execution proceeds before acceptance; done means Plan mode consistently waits for explicit acceptance before starting autoimplementation, with a regression test covering the behavior.
