Plan mode has become unreliable

### Describe the bug

The last couple of weeks planmode starts executing the plan randomly. Sometimes it waits for acceptance, as it is supposed to, but other times it asks me but continues straight away without waiting for answer. I don't see any pattern in it.

I'd expect Plan mode to always **_ONLY_** create a plan

Plan, execute, plan etc. The it will happen at somepoint

### Expected behavior

Plan mode always waits for accept before starting autoimplement

L’issue ne nomme aucun fichier ni test. Reproduisez la séquence Plan/execute/plan sur la version 1.0.57 et observez si l’exécution se poursuit avant l’acceptation ; le travail est terminé lorsque Plan mode attend systématiquement une acceptation explicite avant de démarrer l’auto-implémentation, avec un test de régression couvrant ce comportement.
