Plan mode has become unreliable

### Describe the bug

The last couple of weeks planmode starts executing the plan randomly. Sometimes it waits for acceptance, as it is supposed to, but other times it asks me but continues straight away without waiting for answer. I don't see any pattern in it.

I'd expect Plan mode to always **_ONLY_** create a plan

Plan, execute, plan etc. The it will happen at somepoint

Plan mode always waits for accept before starting autoimplement

Das Issue nennt keine Dateien oder Tests. Reproduziere die Plan/execute/plan-Sequenz auf Version 1.0.57 und beobachte, ob die Ausführung vor der Annahme fortgesetzt wird; abgeschlossen bedeutet, dass Plan mode konsistent auf eine explizite Annahme wartet, bevor die automatische Implementierung gestartet wird, mit einem Regressionstest, der das Verhalten abdeckt.
