TOC superimposed on PDF output [Bug]

**Describe the bug**
The output generated contains the table of contents data superimposed on each page.

Steps to reproduce the behavior:

**Expected behavior**
Table of contents occurs once at start of doc.

- OS: Windows 11
- Docfx version: 2.75.1

Start by reproducing the PDF output on Windows 11 with the attached docfx.json using Docfx 2.75.1, then compare the generated pages with the expected single TOC at the document start. Trace the PDF generation path to identify why the TOC is repeated, and verify that the output contains the TOC only once.
