TOC superimposed on PDF output [Bug]

**Describe the bug**
The output generated contains the table of contents data superimposed on each page.

Steps to reproduce the behavior:

**Expected behavior**
Table of contents occurs once at start of doc.

- OS: Windows 11
- Docfx version: 2.75.1

まず、添付されたdocfx.jsonを使用し、Docfx 2.75.1でWindows 11上のPDF出力を再現します。次に、生成されたページを、ドキュメントの先頭にある想定される単一のTOCと比較します。PDF生成パスを追跡してTOCが繰り返される理由を特定し、出力にTOCが1回だけ含まれていることを確認します。
