VS Code and Insiders apps should use coder-remote extension URIs

Currently they just use the SSH extension directly rather than the coder-remote extension.

The coder-remote extension requires `owner=xyz`, and if you supply `owner=me` it will create a new history record.

VS Code および Insiders アプリの URI 構築箇所を特定し、SSH 拡張機能がどのように選択されるかを追跡します。coder-remote 拡張機能の URI が owner=me ではなく owner=xyz を使用していることを確認し、両方のアプリが新しい履歴レコードを作成せずにそれを使用すれば作業完了とします。
