VS Code and Insiders apps should use coder-remote extension URIs

Currently they just use the SSH extension directly rather than the coder-remote extension.

The coder-remote extension requires `owner=xyz`, and if you supply `owner=me` it will create a new history record.

Locate the URI construction for the VS Code and Insiders apps and trace how the SSH extension is selected. Verify the coder-remote extension URI uses owner=xyz rather than owner=me, and consider the work complete when both apps use it without creating a new history record.
