VS Code and Insiders apps should use coder-remote extension URIs

Currently they just use the SSH extension directly rather than the coder-remote extension.

The coder-remote extension requires `owner=xyz`, and if you supply `owner=me` it will create a new history record.

VS Code と VS Code Insiders の拡張機能 URI を構築する macOS のコードを見つけ、現在の SSH 拡張機能の直接使用と coder-remote の要件を比較します。両方のアプリが owner=xyz を指定した coder-remote の URI を使用し、owner=me による意図しない履歴レコードを作成しなくなれば、変更は完了です。
