VS Code and Insiders apps should use coder-remote extension URIs

Currently they just use the SSH extension directly rather than the coder-remote extension.

The coder-remote extension requires `owner=xyz`, and if you supply `owner=me` it will create a new history record.

Find the macOS code that constructs the VS Code and VS Code Insiders extension URIs, then compare its current direct SSH extension usage with the coder-remote requirement. The change is complete when both apps use coder-remote URIs with owner=xyz and no longer create an unintended history record from owner=me.
