Reproduction steps

1. Start a new Command Code thread.
2. Select Muse Spark 1.3
3. Ask AI to read >50 images (or +50 image PDF)
4. Error occurs: Error: 400 request contains 54 images, exceeding the maximum of 50 allowed per request

Expected behavior

The last tool call result should be stripped out of the context and the error description should be passed to LLM, so the loop doesn't break and the AI knows how to work around these provider limits.

Actual behavior

The error is fatal and the model's run stops. Trying to start the conversation with "Proceed" shows the error
Something went wrong. The agent stopped unexpectedly
The reason for this is because the context still contains over 50 images, and restarting send back all the context in the request, hitting the same error.
