Data: Tool - populate

### Action Items
- [x] Create a json file with id and name for the tool. See phases_data.json as an example
- [x] Create a script named 000[x]_phase_seed.py in data/migrations folder, where x is one more than the highest sequenced script in the folder
- [x] Using 0004_populate_phase as a model
- [x] create a run function that calls update_model_from_json with the model class and file location
- [x] create the migration class that calls run
- [x] update dependencies to specify the previous script

### How to Manually Verify
- Run `source ./start-dev.sh` from django_root directory
- Log into kb app
- Navigate to Tool screen
- Verify records appear

### Automated Tests
This will be done as part of a different issue.
