Add an "end of session" hook that can run a process/skill

Need a way to auto run a process or skill at the end of a session pre /clear command

Proposed solution

Benefit it if you want to use the hook to run a standard list of tasks at the end of a session i.e. to capture what was done in the session. Today this is manually kicked off prior to running the /clear command

/clear コマンドと、既存の hook または skill の実行エントリポイントをすべて追跡することから始めます。/clear の前に実行され、プロセスまたは skill を呼び出せるセッション終了時 hook に必要な動作を定義します。設定されたワークフローがその時点で自動的に実行されれば完了です。
