In a session, I see the last line as "Asking user" but no question prompt has popped up in that session, I just see the regular chat input box.

The insights view shows "Ask user" as still running in the primary session:

Start in the /chat path and reproduce the session state shown in the screenshots, using the reported app version and Windows environment as context. Trace the "Ask user" state between the regular chat input and the insights view; done means the question prompt is visible when the session reports "Asking user" and the activity is no longer incorrectly shown as running.
