HTTP MCP servers disappearing mid-session

Halfway through a session HTTP based MCP server disconnect and we can't get them back

### What happened?

During a session where http based MCP servers are being used they suddenly disconnect and the tools are removed from the session. Note, this is different to http auth failures which is another problem I'm having

Because these tools are just _gone_ I have to ask the agent to spawn a nested session to use them

### Steps to reproduce

1. Start a chat and use HTTP OAuth MCP tools
2. Keep using them over time
3. They stop working

### Expected behavior

MCP servers don't disappear from the agent - or we have a way to re-add tools to the agent
