- the files panel showed an older ( looks like cached ) content of the files, not actual one. But not for all files.
- Terminal and active editor showed the current on-disk file without it.

Start by reproducing the issue in the Copilot desktop app's Files panel with the reported macOS and app versions. Compare the panel contents with the current on-disk file and active editor; done means the Files panel consistently shows current content rather than stale content.
