Scanning too many urls at once causing failure creating issues

We scanned 11 urls. The scanner is now putting the url of every page being scanned in the title of issues. When the workflow found one issue on all 11 urls, it is errored with a 'title too long' error trying create the issue.

Start by reproducing the scan with 11 URLs from the attached workflow output, then trace how the scanner builds GitHub issue titles. Done means the scan can create an issue for a finding shared across many URLs without triggering GitHub's title-too-long error.
