Support source links for a profile mode app.

Right now, these links are disabled for a couple reasons:
1) in profile mode, we do not initialize the script manager. This code should be refactored so that the script manager itself is responsible for updating scripts on isolate changes and initial connection, instead of the current impl which has consumers of the isolate manager (debugger related features) responsible for refreshing scripts on the isolate manager.
2) in profile mode, the debugger page is disabled, so we do not have a destination for the links. We should use the new "tools" stream to navigate to the source uri in the user's IDE instead.

Trace how profile mode initializes the script manager and how consumers of the isolate manager refresh scripts, then inspect the new tools stream navigation path. Done means profile-mode source links open the source URI in the user's IDE and scripts update on isolate changes and the initial connection.
