site: allow users to duplicate templates across different organizations

We should add the organizations dropdown to the duplicate template form so that users can duplicate a template and add it to a different organization:

`/templates/new?fromTemplate={templateId}`

**Requirements**

- [ ] investigate why we use a different form component for this duplicate view - ideally we share the form we use for template creation
- [ ] include organizations dropdown and have dropdown default to the current organization

Start at the duplicate form entry point, `/templates/new?fromTemplate={templateId}`, and compare its form component with the template creation form. Check how the current organization is represented and how organization options are loaded. Done means the shared or equivalent form includes an organizations dropdown that defaults to the current organization and permits duplication into another organization.
