Update Name Request to remove google autofill on all fields

Note: google autofill was turned off on the applicant page where we enter name and address.

- [ ] Remove autofill from the contact details page in the new request flow
- [ ] Check if autofill is in the Edit NR flow on summary page has autofill, if not, disable it

定位新请求流程中的联系详情页面和 Edit NR summary page，然后检查其中的姓名和地址字段如何处理浏览器 autofill。先验证现有 applicant-page 的行为，检查列出的两个流程，并确认在所有需要的地方都已禁用 autofill。
