Update Name Request to remove google autofill on all fields

Note: google autofill was turned off on the applicant page where we enter name and address.

- [ ] Remove autofill from the contact details page in the new request flow
- [ ] Check if autofill is in the Edit NR flow on summary page has autofill, if not, disable it

Locate the contact details page in the new request flow and the Edit NR summary page, then inspect how their name and address fields handle browser autofill. Verify the existing applicant-page behavior first, check both listed flows, and confirm that autofill is disabled wherever required.
