failed to retrieve governance settings — MCP and web tools disabled

### Expected behaviour

A profile is loaded and kiro executes without issue

### Actual behaviour

No profile is loaded and I can login but kiro is not functional

### Steps to reproduce

just login with:
$kiro-cli login --use-device-flow

報告されたエントリーポイントは `kiro-cli login --use-device-flow` です。まずこれを再現し、`q restart` の後に `q doctor` の出力を確認してください。プロファイルが正常に読み込まれ、kiro の MCP ツールと Web ツールが利用可能になれば完了です。
