[BUG] Session becomes unresponsive after 2-3 messages when using Claude Code over remote SSH

### What's Wrong?

When connected to Claude Code via a remote SSH session, the session hangs / stops responding after asking only 2-3 questions in a row. It needs to be restarted to continue working.

### What Should Happen?

The session should remain responsive for extended use over SSH, without hanging after just a few exchanges.

### Steps to Reproduce

1. Connect to a remote machine via SSH.
2. Start a Claude Code session (`claude`).
3. Ask 2-3 questions / send 2-3 prompts in the same session.
4. The session stops responding (hangs) and does not continue processing further input.

Start with the `claude` entry point and reproduce the hang in an Ubuntu/Debian remote SSH session using the VS Code integrated terminal. Send several prompts in one session and compare the behavior with a previous working version if available. Done means the session remains responsive after extended repeated use, with the regression narrowed to a diagnosable cause.
