## Bug
A Claude Code session's title shows correctly in the macOS Claude app but shows a completely unrelated name in the iOS Claude app.
## What happened
- Session started with an auto-generated title.
- Mid-session, the session was renamed twice via the `mcp__ccd_session_mgmt__set_session_title` tool (`session_id: "self"`), both times to the same ticket-prefixed title. Both calls returned success.
- macOS Claude app: shows the renamed title correctly.
- iOS Claude app (same account, same session, checked at the same time): shows "Albert" — a name that has nothing to do with this session or either of its titles, possibly a title bleeding in from a different conversation.
## Expected
Both clients show the current session title, or at worst a stale-but-previous title for this session — never an unrelated one.
## Environment
- Session driven from the iPhone Claude app (Claude Code session), also visible in the macOS Claude desktop app
- Host: macOS (darwin), Claude Code agent session
- Model: claude-opus-5 / claude-fable-5 during the session

Start by reproducing the rename through the mcp__ccd_session_mgmt__set_session_title tool with session_id "self", then compare the same session in the iOS and macOS Claude apps. The issue is done when both clients show the current title and never display an unrelated conversation title; the report offers no file or test entry point.
