Reviews show stale information

**Describe the bug**
pr reviews list bugs that have been fixed by subsequent commits to the PR

issue described on main repo

**To Reproduce**
Steps to reproduce the behavior:

1. create pr with a bug
2. push subsequent commits that fix the bug
3. the github action runs but produces stale output still showing the bug
