💡[FEATURE]: In SOS report Section Improvement by modifying the CSS style and design of the form.

Sorry I haven't checked this issue already exist or not, but if have then assign this to me else if not then consider this request.

**Describe the solution you'd like**
As in the SOS report section in perception the form does not look good. So, I want to modify the design as well the CSS of the form and to add new features if possible.
