track: Code coverage reporting implementation

## Overview
Track the implementation and improvement of code coverage reporting for all hook scripts.

## Current Status
- ✅ Coverage workflow created (`.github/workflows/coverage.yml`)
- ✅ kcov integration configured
- ⚠️ Coverage currently at 0% (no hooks passing tests)

Start by reviewing .github/workflows/coverage.yml, the 6 existing test files, and the dependency issue #27. Check the 26 hook scripts and current 3/26 test coverage, then address the listed shellcheck and test issues. Done means reaching the stated coverage targets, adding a README badge, and configuring coverage as a required PR check.
