Improve subject data requirements flagging for measure calculation

Currently, the subject element of many clinical profiles (e.g., procedure) is not flagged as something used by measure calculation, even though in practice, the link to the specific patient the instance relates to is used. There should be logic in either the MADFSH tool or in the upstream measure bundler process that captures this use so that it gets included in the data requirements for the MADFSH generated IGs.
