Bump azure-storage-queue to 12.0.0 and update docs

The `azure-storage-queue` module was bumped to 12.0.0 (from 2.1.0) yesterday ([pypi history](https://pypi.org/project/azure-storage-queue/#history)). The changes are not backwards compatible.
