We've had community members support gravatars in the past (a big thank you!): https://github.com/microsoft/vscode-pull-request-github/pull/2468
However, I regressed the gravatar behavior a while back: https://github.com/microsoft/vscode-pull-request-github/issues/2771
There's a community PR already out to re-enable gravatar support: https://github.com/microsoft/vscode-pull-request-github/pull/4497
