Description
Could we have a UI similar to GitHub's New Issue Form when creating an issue?
Right now it's a plain md file, but a form with a Title and Description fields would be great.

Research direction
The issue does not name any files, tests, or entry points. Start by locating the VS Code extension flow that creates GitHub issues and compare its current plain Markdown input with GitHub's New Issue Form; done means the creation UI provides separate Title and Description fields.
