Start by reproducing the failure with helloworld.cpp and the supplied launch.json, tasks.jason, and c_cpp_properties.jason on the reported Windows, GCC, and GDB versions. Compare the task output and debugger configuration with the point where the debug adapter terminates; done means the sample program starts a debugging session without unexpected termination.
