When reviewing a PR together (which is a great scenario!), I'd like to have a concise view of all comment threads I'm involved in. And if the agent mentions a comment thread (I was thrilled that it looked over all them and noted that they were addressed), it should be linkable so I can click and see the thread itself.

Start at the /chat entry point and trace how pull request comment threads are represented in the reviewing flow. Done means providing a concise view of threads the user is involved in and making threads mentioned by the agent clickable so they open the corresponding discussion.
