Coverage config hides untested files

`vite/vite.config.ts` sets `coverage.enabled` and an `exclude` list, but not `all: true`. V8 coverage only reports files a test imported, so a source file with no test at all contributes nothing and shows no row. The Cloudflare 95.5% figure is over touched files only.

Files with no matching unit test today, by package: cloudflare 17, deno 21, bun 3, vercel-edge 3, hono 4, effect 2.

**Work item.** Set `coverage.all: true` in the base config and record the real baseline before setting any thresholds. Expect the reported numbers to drop.

Start with vite/vite.config.ts and inspect the existing coverage.enabled and exclude settings. Enable all-file coverage, run the package coverage checks, and record the resulting baseline before adding thresholds; done means untested source files appear in the report and the real baseline is documented.
