I don't know the problem is caused by the sparse matrix or the algorithm, hoping get an explaination.
I run the code in ubuntu18.04 with the latest version of osqp and osqp-eigen. The whole problem and code is in following file.

Research direction

Start with the supplied issure.md reproduction and screenshots, then run it with the reported Ubuntu 18.04, OSQP, and osqp-eigen versions. Inspect the sparse matrices and update-matrix calls involved in the simple QP; done means establishing whether the unstable answer is reproducible and documenting or correcting its cause.
