many and various exercises - add cross references

The C# exercise documentation has many cross-references (identified by (TODO cross-ref-tba)). These will need to be turned into actual references or links once we see what the site looks like.

In addition, there are many cross-references in older exercises that are not so easily identifiable which will need revisiting in a similar way.
