Duplicate tasks can be created

Duplicate tasks can be create currently. This causes issues when trying to mark tasks as completed.
Solution:
- Block creation of duplicate tasks.

No source file, test, or entry point is named. Find the task-creation entry point and inspect how existing tasks are compared; add a regression test if task tests are present. Done means attempts to create an identical task are blocked and task completion remains usable.
