[UX/UI] Create a PoC for our Smart112 solution for people with dissabilities

In the research we have done on supporting people with dissabilities we have found over 20 solutions that could considerably help this vulnerable group. One of these solutions is Smart 112.

**Description:** The elderly and the disabled need special protection measures in the event of a disaster. But one of the problems is that the number and location of these people is not known. This information would be particularly useful in high-risk localities. Before creating an intervention plan, it is important to know the situation of people with disabilities and their needs. A popular method in other countries is to create a registry where disabled people who may need special assistance can voluntarily register or be registered by their relatives. Emergency responders can use this information to make better decisions and improve response times. Family doctors, social workers from town halls and staff from DGASPCs can be among those who can guide people with disabilities to register in the register.

Starting from this description and other information you can find in the dedicated report [that is available here](https://d31wjq819xkxeg.cloudfront.net/Raport-Sanse-Egale-pentru-Persoanele-cu-Dizabilitati.pdf) create a proof of concept of what this solution would look like.

**Steps to take:**
- Create your own figma file and please share it with infrastructure@code4.ro
- Remember we use [Tailwind](https://tailwindui.com/) and [Filament](https://filamentphp.com/) quite a lot. Due to limited resources we prefer to work with libraries that help us implement fast

**To remember:**
- you can either work with simple wireframes or use a free/open-source component library
- we just need a few screens with some dev notes that would indicate behavior or UI elements (see an [example here](https://www.figma.com/file/AcdMESy1oXqkNqjy82a3CD/Centru-Civic-2.0?type=design&node-id=301%3A2&mode=design&t=dhhtj5PnNpI5fV14-1))
- focus on happy flows and the main journey you imagine for the user in the app
- an IA diagram, some user flows, one screen, three screens, 100 screens, we welcome any contribution
