Migrate survey_response natural language keys to ids per question

The language lookup in the survey_response table currently relies on specific English words and phrases translating the same to all other languages regardless of context. This will bite us if we ever add two questions with the same text of english language response but two different meanings based on the contexts of the questions.

Low priority until that event occurs though.
