Survey Template Indeterminate When No Questions Answered

If a user takes a survey, but responds to no questions, we cannot tell what survey they took based on the state stored in the database. This leaves us unable to mark which surveys have been taken and really complicates the logic when trying to return survey template information.

No files, tests, or entry points are named. Start by tracing how a survey with zero answered questions is persisted and how survey template information is returned; done means the stored state identifies the template and supports marking that survey as taken.
