Additional context for server logs

Currently when you output logs in-app, they're not scoped to a specific request ID, so you have to provide additional context yourself. Ideally we provide at least the `id=xxxx` for correlation, if not the `ip=xxx` and others as well.

For now you can use `X-Request-Id` directly (make sure to document).
