Fail on disconnect

Is there/should there be a way to fail a subscription when disconnected from the server?

I would normally crash the process when this happens..

However

const msg = await consumer.receive();
```

just seems to wait forever with debug output indicating retrying but operationTimeoutCeconds having no effect.

Start at the consumer.receive() call shown in the issue and trace how disconnect retries and operationTimeoutCeconds are handled. Determine the expected behavior when the server disconnects, then add coverage demonstrating that a subscription fails rather than waiting indefinitely.
