"Only you see that command's output" caveat is repeated once per tool result in a batch

**What happened**
In batches of six to twelve Bash calls the same caveat sentence was appended six to twelve times in a row, and again once per Read of a persisted output. It adds context tokens without new information.

**Expected**
Once per batch.

**Repro**
Run several Bash calls in one message.

**Environment:** Claude Code CLI 2.1.270 (Claude Code), macOS, auto mode, model claude-fable-5-1, session of 2026-09-14.
