Claude in Chrome extension shows enabled but never connects to session

The Claude in Chrome extension (v1.0.91) is installed and enabled in Chrome
(toggle is on in chrome://extensions), but my Claude session repeatedly reports:

"Browser extension is not connected. Please ensure the Claude browser extension
is installed and running, and that you are logged into claude.ai with the same
account as Claude Code."

Steps already tried:
- Confirmed extension is installed and toggled on in chrome://extensions
- Fully restarted Chrome
- Checked again multiple times after restart — still not connecting
