Reproduce the issue from the Claude Code plugin UI on macOS with accessibility.signals.chatUser.sound enabled, using a prompt that requests a Yes/No choice. Compare version 2.1.266 with the previous working version if available; done means the audio cue plays when user input is required without affecting other system audio.
