[BUG] Files viewed in Claude Desktop do not consistently update when changed.

### What's Wrong?

Using Claude Code in the Claude Desktop application
I have my conversation open and next to it using the "..." -> "Files" menu I have open a file that we're working on.

Claude edits the file but the file preview on the right does not update. It's showing a stale copy.

I have to press F5 to refresh it.

### What Should Happen?

When Claude edits the file, or ideally, when anything edits the file, the view of it should update.

### Steps to Reproduce

- Create a directory
- Open it in claude code, in claude desktop app.
- Ask to create a file (Example.md)
- Open it in the 'Files' side panel
- Have claude keep updating it as the conversation continues
- Watch as sometimes the view of the document doesn't update.

### Additional Information

Incorrect "duplicates" are being detected. This issue is not:
- File-tree not updating
- File diffs not being shown or anything re. "diff viewer"

It does look like a duplication of this original issue: https://github.com/anthropics/claude-code/issues/67971 which says "Please open a new issue if this is still relevant" This issue is still relevant.

PS: I had to select "Windows Terminal" but I'm using ClaudeCode in the ClaudeDesktop app.

Reproduce this in Claude Desktop on Windows using the Files side panel and Example.md, then trace the file-preview refresh path and its file-change notifications. Done means edits made during the conversation consistently appear without pressing F5, while keeping the file-tree and diff-viewer cases separate.
