creating a new page issue

how to add a new page element example(feedback)i have created a new directory under pages as given in the docs of blur admin
and added into the pages.module.js
then created a ui router in command prompt
file mapping in given as per the instruction
but i cant add the required element into the existing template
i had sorted out a issue in index.html/release and nothing is given inside the script there
what should be given in the script or added to add the required page into it
