Manager 上下文溢出崩溃（Compaction 未触发）

Manager 在处理长时间会话时，上下文超过安全阈值后崩溃。关键问题是 compaction 机制未触发（`compactionAttempts=0`），无法自动压缩上下文。

1. Manager 长时间运行，积累约 140 条消息
2. 工具循环（tool loop）多次执行
3. 上下文持续增长超过安全阈值
4. 未触发 compaction（compactionAttempts=0）
5. Manager 崩溃

### 期望行为

1. 修复 compaction 自动触发机制（0 次不正常）
2. 工具循环中提前预警上下文即将溢出
3. 溢出前自动保存关键会话状态并刷新会话
