Start with linked issue #17 and review the existing survey and spreadsheet links. Check the landing-page URL and Google Forms URL, then clarify whether the QR code is required and what the disclaimer and privacy statement must contain. Done means the survey page has the agreed introductory text, disclaimer, privacy statement, survey link or iframe, and any agreed QR code.
