Multiple entries for the same Package and fingerprint

In my own instance of purldb, I notice that there are multiple entries in the `approximate_directory_content_index` and `approximate_directory_structure_index` tables with the same Package and fingerprint. We should find a way to clean up duplicate entries in the tables and update the code to prevent this.
