?v={site_modified} only works on HTML files, not HTM files.

I have htm files that I was using this on, and I realized that this doesn't work on them. Does it work on every index.html for each directory or just index.html in the root of the site's folder?

Start by locating the implementation of the ?v={site_modified} behavior and how it selects index.html files. Reproduce the issue with both .html and .htm files, including files in the site root and subdirectories. Done means the intended behavior is established and the reported HTM case is fixed or clearly documented.
